Each world is an isolated and complete thing, and without sentient life, there would be no connections. But there is sentient life. Since creation, new connections have been forged. Worlds may have Main Gates in cities, which allow travel to other Main Gates.

Of course, it may be possible to find other, smaller doors if one were to look...
