Some LHDs have been giving people out of date forms. :doh: So here is the most current for your use.

Does anyone have a clean .pdf copy of the latest campground Site Evaluation Report (HEA 5228)? The one on the ODH website is a scan of a hard copy and has a revision date of 5/2000, and the only nice clean form we have here has a revision date of sometime in 1993.

Yeah, I e-mailed Lee Ann and I got the same scan that's on the website. I would think that these forms are created in-house somewhere at ODH, and that they could be saved as .pdf files so clean electronic copies would be available for everything? :dunno:
